Coping with Pregnancy Loss and Miscarriage: When Can Counselling Help?

A grief that is often not recognised
Miscarriage, ectopic pregnancy, stillbirth and termination of pregnancy for medical reasons can involve profound feelings of loss. Emotional responses vary considerably, and the stage or circumstances of the pregnancy do not reliably predict how deeply someone will be affected.1
Pregnancy loss is associated with increased symptoms and risk of anxiety and depression compared with people who have not experienced a loss. Partners may also grieve, and two people within the same relationship may experience or express the loss differently.2
Common emotional responses after pregnancy loss
There is no single “right” way to grieve. People may experience:
- Waves of sadness, anger, guilt, or longing
- Difficulty being around babies, pregnancy announcements, or family events
- Anxiety or fear during a subsequent pregnancy
- Changes in relationships as partners process the loss in different ways
- A sense of loss of confidence or trust in their body
- Difficulty concentrating, sleeping, or returning to everyday routines
These responses can be understandable reactions to a significant loss. For some people, feelings gradually become easier to manage with time and support. For others, additional help can make the process less overwhelming.
Research does not suggest that everyone will develop a mental health disorder after pregnancy loss. It does show that anxiety and depression are more common after perinatal loss, making individual assessment important when distress is persistent, severe or affecting daily functioning.2
When can counselling help?
Counselling may be helpful if:
- Feelings of grief, anxiety, or distress remain intense or difficult to manage
- Sleep, work, relationships, or daily life are being affected
- A future pregnancy is accompanied by overwhelming fear or worry
- Previous pregnancy losses are affecting your emotional wellbeing
- You feel isolated or would value a confidential space to talk
- You are unsure how to process what has happened
Seeking support does not mean that you are “not coping.” Counselling may provide space to process the experience, while persistent depression, severe anxiety, trauma symptoms, self-harm thoughts or an inability to remain safe require assessment by an appropriately qualified mental health professional. Immediate safety concerns require emergency help rather than a routine counselling appointment.
What happens in counselling?
Sessions are usually around 50 minutes and provide a confidential, supportive space to explore your experience at your own pace.
Counselling may be offered individually or, where appropriate, with a partner. The approach should be adapted to the person's needs, preferences, cultural context and stage of grieving rather than assuming there is one correct way to respond to pregnancy loss.
The focus is not on forgetting the loss, but on helping you process what happened, make sense of your emotions, and find ways to move forward while honouring your experience.
Confidentiality
Counselling is a confidential service. What you discuss will remain private within professional and ethical guidelines. Any limits to confidentiality will be explained clearly at the beginning of therapy.
