Depression, seen properly
Depression is more than sadness. It changes sleep, appetite, energy and the way you think about yourself. It is also one of the most treatable conditions in medicine when it's addressed properly. The first step is a real assessment — not a five-minute questionnaire — that separates depression from burnout, thyroid problems, low B12, grief, or the effects of alcohol and poor sleep.
Symptoms to watch for
- Low mood most of the day, most days, for more than two weeks
- Loss of interest in things that used to matter
- Sleep that's broken, shortened, or won't end
- Tiredness that isn't fixed by rest
- Appetite or weight changes in either direction
- Guilt, self-criticism, or a sense of being a burden
- Trouble concentrating, decisions that feel impossible
- Thoughts of hopelessness or that life isn't worth it — please seek help urgently
The assessment
A 60-minute first appointment covering your current symptoms, any past episodes, sleep and substances, life circumstances and physical health. We use the PHQ-9 to track severity over time and screen for anxiety, ADHD or bipolar patterns that can be missed. Basic blood tests (thyroid, B12, vitamin D, iron) are arranged when the picture calls for it.
Treatment plan
- CBT, interpersonal therapy or short-term psychodynamic therapy
- SSRIs and SNRIs — first-line for moderate to severe depression
- Careful switches or combinations when the first medication doesn't work
- Sleep, exercise and light exposure — small changes with real effect
- Screening for the medical causes that mimic depression (thyroid, B12, vitamin D)
- Family or partner involvement when it helps, only with your consent
Follow-up
The first month is the most important — that's when medication needs adjustment and therapy needs commitment. We see you at 2 weeks, then 4–6 weeks, then space out as symptoms lift. Coming off medication, when the time is right, is done slowly and planned together.

Frequently asked questions
How do I know if this is depression or just a rough patch?
A rough patch lifts when the situation changes. Depression stays even when things improve, and colours everything. If low mood has been present most days for two weeks and is affecting sleep, work or relationships, it's worth an assessment.
How long will medication take to work?
SSRIs and SNRIs usually take 4–6 weeks to reach their full effect. Side effects, if any, are strongest in the first week or two and often settle. We check in at 2 weeks and again at 6 weeks.
Do I have to stay on medication forever?
No. For a first episode we typically continue for 6–9 months after recovery, then taper. For recurrent depression, longer treatment is often the right choice — we review this with you.
Is confidentiality guaranteed?
Yes. Records are private and are not shared with employers or immigration. Insurance disclosures are limited to what's clinically necessary, with your consent.
