It's one of the great paradoxes of life in Dubai: we live in one of the sunniest cities on Earth, yet up to 90% of UAE residents are estimated to have insufficient vitamin D. It's now one of the most underdiagnosed drivers of joint pain, muscle weakness and fragile bones we see in the clinic.

Why deficiency is so common in the UAE

  • Air-conditioned indoor lifestyles during peak sun hours
  • Tinted car and building windows that block UV rays
  • Sunscreen and cultural clothing that reduce skin exposure
  • Diets low in oily fish, eggs and fortified dairy
  • Darker skin tones need more sun to make the same amount of vitamin D

What vitamin D actually does

Vitamin D helps your gut absorb calcium and phosphate — the minerals your bones and muscles need. It also supports immune regulation and mood.

When levels are low, calcium absorption drops. The body pulls calcium from bone to keep blood levels normal, weakening the skeleton over time.

Signs of deficiency to watch for

  • Dull, aching bone pain — often in the lower back, hips or ribs
  • Muscle weakness or heaviness, especially in the thighs
  • Unexplained fatigue
  • Frequent muscle cramps
  • Joint pain that doesn't fit a clear pattern
  • Low mood or seasonal-style depression
  • Frequent infections

Vitamin D deficiency doesn't only affect bones. Low levels are linked with:

  • More severe symptoms in rheumatoid arthritis and lupus
  • Worse pain and stiffness in osteoarthritis
  • Increased risk of tendon and muscle injuries
  • Slower recovery from joint surgery

Long-term deficiency causes:

  • Osteomalacia in adults — soft, painful bones
  • Rickets in children — bowed legs and skeletal deformity
  • Osteoporosis — silent bone thinning that only shows up when a fracture happens

How it's diagnosed

A simple blood test measures 25-hydroxyvitamin D. Results are typically classified as:

  • Deficient: less than 20 ng/mL
  • Insufficient: 20–29 ng/mL
  • Sufficient: 30–50 ng/mL

If levels are very low or your bones ache, we may also check calcium, phosphate, parathyroid hormone and a DEXA scan to look at bone density.

Treatment

  • Short-term loading doses of vitamin D3 (cholecalciferol) — prescribed based on your level
  • Daily maintenance — typically 1,000–2,000 IU/day for most adults
  • Calcium — through diet first (dairy, leafy greens, sardines, fortified foods) and supplements if needed
  • 15–20 minutes of daylight on arms and face several times per week
  • Retest in 3 months to confirm you're in the target range

When to see a rheumatologist

If joint or bone pain persists despite correcting your vitamin D, or if there's a family history of osteoporosis or autoimmune disease, book a rheumatology review. We often find that vitamin D deficiency is only part of the picture.
